admin, publish it, add it to any page of your club site. Waivers, volunteer sign-ups, show
entries, surveys, RSVPs. Every response lands in one table — searchable, sortable, exportable
to CSV.

Consent checkboxes record who accepted and when, so you have a real answer before the
ride leaves. And responses belong to the club, not to whoever built the form — officers
change, the data stays.
